Account Application

This form provides us with all the information needed to open your new account.  Please include a clear and legible copy of a government-issued photo identification (eg. driver’s license or passport).  Also, please include a check for the Annual IRA Custodial Fee.

Beneficiary Designation

This form is required if you have beneficiaries.  Please indicate your beneficiary designations making sure to include the percentage attributable to each.  Note that the totals in each beneficiary category (primary/secondary) must equal 100%.

Checkbook IRA LLC Agreement

This form is required when requesting an investment in an IRA LLC. Please send the completed form along with the Investment Authorization forms.

Investment Authorization – Custodian Held Assets

Use this packet to request investments in anything other than an IRA LLC.

Investment Authorization – IRA LLC

Use this packet to request an investment in an IRA LLC ONLY.

This packet contains the Investment Authorization form, instructions on how to complete the form and document requirements for investing in an IRA LLC.  Please review the packet in its entirety and submit the required documents with the completed Investment Authorization form.  If sending funds by wire, please also complete and return the Wire Request Form.

IRA Asset Valuation

Use this form to report the fair market value of the assets held in your IRA custodial account.

IRA Asset Valuation - Required Minimum Distribution
